Essential FSO Forms and Policies and Procedures


Essential FSO form and policies and procedures assist the FSO with administrating NISPOM requirements. These forms are packaged as an assembled group for your convenience. The forms are also tailorable to fit your individual or organization needs and configuration requirements.

Once downloaded, add your business information and logos. Just purchase and download the content:

1. The Foreign Travel Form: All cleared employees are required to report foreign travel to the FSO. The FSO files foreign travel through Defense Information System for Security (DISS). Provide the forms to the employee for them to populate. Once populated, they can provide to the FSO with the appropriate and required DISS fields. This saves hours and days of emailing requirements by providing all of the information at one time.

2. The Facility Security Officer (FSO) Appointment Letter: The FSO is required to be a U.S. citizen, have a security clearance at the level of the facility, an employee of the company and appointed in writing. This form meets appointment requirements and once populated and signed, can be uploaded into the National Industrial Security System (NISS). Use this form each time an FSO is appointed and each time a Change Condition Package requires it.

3. The Insider Threat Program Senior Official (ITPSO) Appointment Letter: The ITPSO is required to be a U.S. citizen, have a security clearance at the level of the facility, an employee of the company and appointed in writing. This form meets appointment requirements and once populated and signed, can be uploaded into the National Industrial Security System (NISS). Use this form each time an ITPSO is appointed and each time a Change Condition Package requires it.

4. The Security Clearance Justification and Request Form: This form is required to completed for each employee prior to gaining access to classified information. It should be used to either initiate a new security clearance request, initiate a security clearance reinvestigation, or when a new employee’s clearance must be owned or serviced by our organization in DISS. This form allows your company to demonstrate NISPOM compliance where: the contractor shall limit requests for personnel clearances to the minimum number of employees necessary for operational efficiency, consistent with contractual obligations.

5. Insider Threat Program Policy: This document demonstrates your company’s establishment of a program to protect classified information, residing in personnel knowledge, equipment, networks, or systems from insider threats. An insider threat is the likelihood, risk or potential that an insider will use his or her authorized access, willingly or unwillingly to do harm to the security of the United States. These threats may include harm to the program information to the extent it impacts obligations to protect classified national security information

6. Security Executive Agent Directive 3 (SEAD 3): This document demonstrates your company’s establishment of a program to address reporting requirements. An The FSO will provide the following baseline reports upon occurrence. Cleared employees should understand their critical responsibility to report behavior or activities of those around them that could compromise classified information, workplace safety, and/or our National Security. Formats for completing reports can be found in Appendix A.

7. Standard Practice Procedures: This Standard Practices and Procedures (SPP) describes your company’s policies regarding the handling and protection of classified information. This SPP is applicable to all employees, subcontractors, consultants, vendors, and visitors to our facility and is a supplement to the National Industrial Security Program Operating Manual (NISPOM) Download this from and tailor to fit your company.
